Doc2Form

Doc2Form turns existing documents into native Google Forms. Upload a PDF (including scanned PDFs), a Word .docx file, or a JPG/PNG image of a paper form, or just describe the form you need in plain English. Gemini AI finds the questions, answer options, field types, and section breaks. Doc2Form then uses the Google Forms API to create a real, editable form in your own Google Drive, usually in under 60 seconds and with no copy-paste.

What problem does it solve? Many teams still keep their forms as PDFs, Word files, or printed paper. Rebuilding those forms by hand in Google Forms means retyping every question, recreating every multiple-choice option, choosing field types one by one, and splitting the form into sections. Doc2Form does that work for you: you upload the document, and a few moments later the finished Google Form is waiting in your Drive, ready to edit, share, and collect responses.

It reads messy scans and photos that plain-text tools miss. If all you have is a phone photo of a paper form or an old scanned PDF, Doc2Form can still extract the questions and options.
